ChamberComm will serve as your Local Area Long Distance Provider (Local Toll Calls). Local Toll Calls span greater distances than local calls, but fall short of being long distance calls. Local Toll Calls are sometimes referred to as regional toll, shorter distance or local long distance calls. Selecting this option will allow your Local Toll Calls to be billed by Chambercomm at Chambercomm Instate Rates.

Thank you for choosing ChamberComm as your long distance provider. We are absolutely certain that you will notice the superior quality of our carrier's 100% digital fiber optic network.

By clicking the "submit" button above, I hereby authorize ChamberComm's selected carrier, US Fibercom to provide long distance services on the phone line(s) or other services listed in this Agreement and to act as my agent in all matters related to providing the services, including intra-lata service. I understand that I may select only one primary long distance carrier for each calling area for each number. I further agree that should I decide to terminate service with US Fibercom, I must provide US Fibercom at least thirty (30) days notice prior to termination. I attest that I am the person authorized to order services on the phone line(s) listed in this Agreement. Service is provided under terms, conditions and rates set forth in applicable tariffs, which are subject to change. ChamberComm or its authorized carrier shall not be liable to any person or entity for any special, incidental or consequential loss or damages. This Agreement and the current tariffs of US Fibercom constitute the entire agreement between the parties. ChamberComm pre-subscribed customers can cancel their service by contacting their local telephone carrier, whose phone number is listed on their local telephone bill. Additional help and resources are available by calling ChamberComm Customer Service at 800.323.4587. I understand that there may be a one-time charge by my local phone company to change long distance carriers. I also understand that I can be held responsible for any fraudulent usage. I guarantee US Fibercom the faithful payment, when due, for all charges incurred for services provided together with all pertinent federal, state, and local taxes. If payments are not made when due and attempts made by US Fibercom to collect such payment are not successful, regardless of the type of billing selected, customer's credit card or checking account may be charged without further notice to customer. I further understand that if valid credit card authorization is not submitted with this order and my account is past due, US Fibercom reserves the right to terminate service. For E-Bill customers: By submitting my valid credit card information, I hereby authorize all monthly charges for services listed above to be charged on the above listed credit card or electronically debited from the above listed checking account. These charges will be billed by US Fibercom or its affiliates. My credit card or bank statement will read US Fibercom or affiliates for all charges or debits. The aforementioned authorizations shall remain in effect until cancelled in writing.

A Universal Service Fund Charge (USF) will apply to your monthly recurring/usage charge, interstate and international usage. This charge started on January 1, 1998 as part of the FCC overhaul of telephone fees. All companies that provide telephone service between states pay a set percentage of their previous year's billings. The charge is designed to ensure affordable access to telecommunications services for telephone customers with low incomes, telephone customers who live in areas where the cost of providing telephone service is extremely high, libraries, schools, and rural health care providers. All other required local, state, and federal taxes will also apply.

ChamberComm will credit PIC Change charges - one-time charges made by your local phone company to change long distance carriers - (up to $5.00 per line) to your account after service has been active with US Fibercom for at least ninety (90) days. All customers MUST present their bill for proper credit to be issued.
